Request

Requests are used when visitors want to ask you or page administrators questions or to make contact. When the request form is submitted, notification email is sent to admin with the information filled by user. If 'Enable Auto-Reply' is selected (see below), an email is also sent to the person who submitted the form.

Settings

• Send Notification to (edit box) - The email address the form data will be sent to. Set multiple email addresses, using (;) as delimiter.
• From (edit box) - The email address the form is sent from. Use email address with same domain name as your server, for example: admin@yourdomain.com
• BCC (edit box) - The email address the form should be sent to as BCC (blind CC, someone who privately sees a copy of the submitted form). You can set multiple email addresses, using (;) as delimiter.
• Use Sender email address in 'From' (check box) - Enable if you want to use the visitor's email address as 'From' address when sending the form. Note, that this option is not supported on all servers.
• Enable Auto-Reply (check box) - The auto-reply function will send e-mail to visitor of your site when form is successfully submitted.
• Admin Notification (button) - Set the subject and message for the notification email. You can use form field macros in both fields. Example: Visitor %Name% submitted request. Default message contains %FORM_DATA% macro and will be replaced with list of all form field values. If you want to include only some of them, remove %FORM_DATA% and use form field macros (%Name% , %Email%, %Place%, etc).
• Auto-Reply E-mail (button) - Sets the subject and message for the auto-reply email. You can use form field macros in both fields (%Name%, %Email%, %Place%, etc).
• Submit Message (edit box) - Set a message that will be displayed to user after he submits the form.

NOTE: Don't forget to test the form yourself after adding it to your page!
